SPA POLICIES AND CODE OF CONDUCT

LMLA is devoted to enhancing overall well-being through a variety of professional services that encourage the renewal of mind, body and spirit. To enjoy your spa experience to the fullest, please observe our Polices and Code of Conduct. We ask that all clients, as well as staff, act responsibly and respectfully to help ensure your satisfaction, comfort and safety, as well as that of others.

SPA POLICIES

Card On File + Deposits Policy

In order to secure your time and ours, an email and card must be on file to book any service.

  • All services up to $50 are required to be paid in full at the time of booking.

  • All services $51+ require a 50% deposit at the time of booking.

  • All Deposits are NON-REFUNDABLE!

  • All Deposits are TRANSFERABLE up to 1 time ONLY in a 12 month calendar!


* LMLA reserves the right to request a deposit for any service at our discretion

Clients MUST give a 24 hour advance notice to cancel or reschedule an appointment for services that are LESS than 60 minutes in the duration of being serviced. Clients who give notice -before 24hrs- to cancel or reschedule an appointment, can TRANSFER their appointment/deposit ONE TIME ONLY in a 12 month calendar. Late Cancellations thereafter -after 24hrs- for these booked appointments will be charged a 25% Non-Refundable Late Cancellation fee of booked service/s cost. -NO EXCEPTIONS

ALL No-Call/No-Show, Same Day cancellation/rescheduled appointments will be charged a 25% Late/No-Show Fee of booked service/s cost -NON-REFUNDABLE! This fee will be applied to the card on file. A payment of previously missed appointment must also be paid in full before booking again with LMLA. This also applies for same day cancellations/reschedules for services that are 60 minutes or more in the duration of being serviced. Last minute cancellations are appointments canceled or rescheduled the day of booked appointment. -NO EXCEPTIONS

NOTICE- All declined cards, no-call/no-show & last minute cancellations, will be required to pay in full their previous canceled appointment before booking with LMLA and service is NOT transferable. -NO EXCEPTIONS

After 3 occurrences of no-call/no-show, last minute cancellations, same day cancellations/reschedules, declined cards, you will no longer be available to book ANY services with LMLA/LMLA Wellness.

YOUR RIGHTS AND RESPONSIBILITIES AS SPA GUEST

AS A SPA GUEST IT IS YOUR RESPONSIBILTY TO:
  • Communicate your preferences, expectations and concerns.

  • Communicate complete and accurate health information and reasons for your visit.

  • Treat staff and other guests with courtesy and respect.

  • Use products, equipment and therapies as directed.

  • Adhere to the spa’s published policies and procedures.

AS A SPA GUEST YOU HAVE THE RIGHT TO:
  • A clean, safe and comfortable environment.

  • Stop a treatment at any time, for any reason.

  • Be treated with consideration, dignity and respect.

  • Confidential treatment of your disclosed health information.

  • Ask questions about your spa experience.

  • Information regarding staff training, licensing and certification.

  • Trained staff who respectfully conduct treatments according to treatment protocols and the spa’s policies and procedures.

PRIVACY POLICY

  • To respect the purpose for which our site visitors and others gave us email information. We give users choice and consent over how their personal information is used, and remove names immediately upon request.

  • To not sell, rent or loan our subscriber or customer lists to third parties for email, direct marketing or other purposes. We never allow third parties any other use of these subscriber lists.

  • To send notices of appointments or special offers to our clients who request them (opt-in). LMLA will immediately unsubscribe those who request it through convenient unsubscribe links in every email or by mail to our address which is prominently displayed.

  • To store subscriber and password files with personal information in a secure manner. We store such files in areas not accessible to Web browsers, behind password protection or offline.

  • For your privacy LMLA DOES NOT hold any credit/debit card information.

  • To continue this policy beyond the termination of our company. If a majority portion of the assets of our company, which might include our subscriber and other lists, were to be acquired by another organization, it would be done only as part of a clear agreement binding that organization to abide by the spirit and letter of our privacy policy.

PERMANENT JEWELRY POLICIES

  • ​​All permanent jewelry sales are final after we custom fit, cut the chain and weld it on you.

  • ​Clients MUST give a 24 hour advance notice to cancel or reschedule their permanent jewelry appointment. Clients who give notice -before 24hrs- to cancel or reschedule an appointment, can TRANSFER their appointment/deposit ONE TIME ONLY in a 12 month calendar. Late Cancellations thereafter -after 24hrs- for these booked appointments will be charged a 25% Non-Refundable Late Cancellation fee of booked service/s cost. -NO EXCEPTIONS

  • To ensure that you have a positive LMLA permanent  jewelry experience, we offer a 60-day warranty:   

​

  • Within the first 60 days if the welded product you purchased from us becomes un-welded, detached, or needs to be refitted, for any reason, and you have the product with you, you can schedule a repair appointment with us and have it welded back, or adjusted, for free. 
    This service is free and unlimited within 60 days of initial purchase.
    After 60 days of initial purchase a service fee of $20 will be applied per item welded or resized.  This fee is waived with the purchase of another LMLA permanent jewelry at the time of appointment.

  • Our 60 day warranty does NOT cover lost items, scratches that come with normal wear and tear (like a bent charm or broken chain), improper use or storage of your product, and any modification to your product done by a third party outside of LMLA.
